Booster activities is the active participation in the work of the V.F.W. at all Levels. This Form is a Supplemental Report Form. It is to be used in conjunction with the V.F.W. Booster Report Form for V.F.W. Recruitment.
For each group; A, B, C, D and E ( On Back):
Number of Cooties Working:
Compound the Number Working. Examples: If one Cootie worked one activity, then Cootie Work = 1. If one Cootie worked 4 Bingo Games, then Cootie Work = 4. If five Cooties worked one Funeral, then Cooties Work = 5. If four Cooties each worked three Breakfast, then Cooties Work = 12.
Number of Events / Activities:
Compound the Activities: Examples: If one Cootie worked one Activity, then Activities=1. If one Cootie worked four Bingo Games, then Activities = 4. If five Cooties worked one Funeral, then Activities = 5. If four Cooties each worked three Breakfast, then Activities = 12.
For Group G:
Value of newly purchased items would be cost. Value of used or other items that was purchased prior, would be market value.
